Last 3 years of Income Tax Returns (Individual and Corporate)
Last 3 years of W2s, 1099s, Unemployment Benefit statements
Most recent three months of paycheck stubs showing hours worked, rate per hour, total gross pay, itemized deductions and net pay and year to date figures
Copies of last 3 months of credit card statements, loan statements, and any other unsecured or secured debts that you pay on a monthly basis, showing total outstanding balance due and minimum monthly payment and creditor.
Copy of the deed granting you or your spouse an interest in all real estate owned, including homestead, showing the complete legal description for all real property.
Most recent mortgage statement showing total outstanding balance due on the mortgage and monthly payment amount.
Copies of last 3 months of all checking and savings accounts, whether individual or joint accounts, bank statements, check registers and cancelled checks.
Copies of most recent statements showing total value of all IRAs, Money Market Accounts, Brokerage or Stock Market Accounts or all such other financial accounts.
Copies of all company defined benefit Pension Plans through employer showing total value.
Copies of most recent 401(k) savings plan showing total present value, through your employer and or your spouse’s employer.
Make a list of all motor and recreational vehicles owned by either you and or your spouse, stating year, make and model, whether it is subject to a loan, the loan balance, whether the loan is joint or individual, the monthly payment and which party wants or uses the vehicle.
A list of all such other items of household goods and furnishings and personal property including guns, tools, equipment or such other item that has a value of $300 or greater, and indicate whether you are requesting to be awarded the item or whether the other party can have the item and for each item, state your opinion of the present value.
A copy of any health and dental insurance coverage plan that you currently have available through your employer or your spouse’s employer, and verification of the monthly cost thereof.
Copies of any life insurance policies insuring you or your spouse’s life.
An itemized list of your monthly living expenses, including rent, mortgage, homeowners insurance, heat, utilities, internet, TV, phone, food, automobile payment, automobile gas, automobile insurance, entertainment, going out to eat, children’s costs, clothing, laundry, monthly debt payments on credit cards and other loans, personal expenses for hair cuts and personal items.
If you are claiming that you owned any asset prior to the marriage, or that you inherited any item during the marriage, provide all documentation showing the same, and the value of the item.
A copy of your resume or work history, employer names and rate of pay upon leaving such employer and positions you have held.
